Do you take insurance?
Major insurances are accepted. To determine your eligibility for services under your insurance plan, please call 515-237-5045 in Des Moines or 712-328-3086 in Council Bluffs.
What if I don't have insurance?
If you do not have insurance, we can provide service on a sliding fee scale based on annual income and number of dependents. Cost ranges from $10 to $125 per visit.
